SPECIFICS:

My House Fitness®: Plano and Client shall agree upon the date and time of personal training sessions ("Sessions") in accordance with the studio hours of operation, and at the rate set forth in the individual contract. Sessions will not commence until Client provides signed My House Fitness®: Plano Waivers and complete our Medical Questionnaire. As any new exercise program, all Clients should consult with their family doctor before starting a new exercise program. 

 

PAYMENT POLICY:

Payments for training sessions are paid in monthly payments. All month-to-month clients are required to complete an Automated Credit Card Payment Agreement. My House Fitness® Inc. will process payments on clients due date. Insufficient funds, bounced checks or denied credit cards will be charged $25.00. If clients paying with cash or check are more than 3 days overdue, a late payment of $10.00 will be assessed. If Client's payment is more than (2) two weeks overdue, Trainer may suspend Sessions until Client's withstanding balance is current. 

 

WARDROBE & PUNCTUALITY:

Client shall be dressed in cool, comfortable and dry athletic wear and athletic shoes (closed-toed). We keep our training studio freshly stocked with all the mats + towels you need. Clients shall be ready to train at the specified time between My House Fitness®: Plano and Client. Failure to be prepared to train may result in shortened workout or possible cancellation of the Session. If Client anticipates running late, he/she should contact Trainer as soon as possible. If Client is late, a full Session is NOT guaranteed; My House Fitness®: Plano will attempt to accommodate late Clients, but is not responsible for a shortened session due to lateness. A late session on behalf of Client is still considered a full, payable session. 

 

CANCELLATION OF INDIVIDUAL SESSIONS:

(24) Twenty-four hour cancellation notice by phone, email or text is required for rescheduling or cancelling any and all individual Sessions. Any and all cancellations with less than 24-hours notice will result in forfeiture of the Session - without refund. If My House Fitness®: Plano must cancel a Session, he will do so by phone, email or text with at least (24) twenty-four hours notice or Client will receive a complimentary Session for his/her inconvenience.  Early cancellation of monthly training packages may be required to pay a $50.00 early cancellation fee.

 

OBSERVANCE OF HOLIDAYS:

My House Fitness®: Plano will observe the following holidays and will be closed: 

•Labor Day
•Memorial Day
•Independence Day (July 4th)
•Thanksgiving
•Christmas Eve
•Christmas Day
•New Year's Day (January 1st)

 

CANCELLATION + REFUND NOTICE:

Notice to Purchaser: do not sign any contract until you've read it or if it contains blank spaces.

Please refer to My House Fitness®: Plano Training Agreement provided at the studio for all specific terms and conditions.  


TESTIMONIAL + MARKETING POLICY:

My House Fitness®: Plano leverages real client results for promotional and marketing material. Becoming a Client grants My House Fitness®: Plano permission to the rights of Client's image, likeness and sound of voice as recorded on audio or video without payment or other consideration. Client images and video may be edited, copied, exhibited, published or distributed and all rights to inspect or approve the finished product wherein Client likeness appears. Rights to royalties or other compensation arising or related to the use of Client images or video are waived upon effect of Client contract.
